Je peux bri\u00e8vement me pr\u00e9senter et pr\u00e9senter mon r\u00f4le dans la strat\u00e9gie de lutte contre la r\u00e9sistance aux antimicrobiens de Chypre.<\/strong><\/p>

Markella Marcou<\/strong>Je suis le chef du d\u00e9partement de microbiologie de l'h\u00f4pital Archev\u00eaque Makarios III, qui est l'h\u00f4pital de p\u00e9diatrie, d'obst\u00e9trique et de gyn\u00e9cologie de Nicosie, et je suis \u00e9galement coordinateur de notre comit\u00e9 AMS nouvellement cr\u00e9\u00e9. Je travaille un jour par semaine au minist\u00e8re de la Sant\u00e9, o\u00f9 je collabore avec Linos sur des programmes nationaux de r\u00e9sistance aux antimicrobiens et de pr\u00e9vention des infections.<\/p>

Linos Hadjihannas<\/strong>Je dirige le service des maladies infectieuses de l'h\u00f4pital g\u00e9n\u00e9ral de Nicosie et je travaille avec Markella au minist\u00e8re de la Sant\u00e9. Nous sommes tous deux des points focaux pour la r\u00e9sistance aux antimicrobiens et les infections nosocomiales.<\/p>

Qu'est-ce qui a motiv\u00e9 votre int\u00e9r\u00eat pour la r\u00e9sistance aux antimicrobiens (RAM) et la gestion des antimicrobiens (stewardship) ?<\/strong><\/p>

Linos<\/strong>: Les bact\u00e9ries multir\u00e9sistantes (BMR) et les infections nosocomiales sont au c\u0153ur de nos domaines. Nous constatons directement les risques pour la s\u00e9curit\u00e9 des patients, ainsi, contribuer aux efforts nationaux de lutte contre les BMR est une extension naturelle de notre travail. Chypre a une consommation \u00e9lev\u00e9e d'antibiotiques et des taux de BMR \u00e9lev\u00e9s, similaires \u00e0 d'autres pays d'Europe du Sud. Ce n'est pas qu'un probl\u00e8me national, c'est culturel.<\/p>

Marcelle<\/strong>Exactement. Il s'agit d'habitudes \u2014 la fa\u00e7on dont les antibiotiques sont prescrits et dont la pr\u00e9vention des infections est pratiqu\u00e9e. Nous ne manquons pas de lignes directrices, mais le suivi et le respect sont les vrais obstacles. Jusqu'\u00e0 r\u00e9cemment, la plupart des h\u00f4pitaux n'avaient m\u00eame pas de comit\u00e9s de gestion des antimicrobiens. Cela a chang\u00e9 l'ann\u00e9e derni\u00e8re, lorsqu'une circulaire du minist\u00e8re les a rendus obligatoires.<\/p>

Pourquoi a-t-il \u00e9t\u00e9 n\u00e9cessaire de rendre obligatoires les comit\u00e9s d'am\u00e9lioration de la qualit\u00e9 (AMS) dans les h\u00f4pitaux ?<\/strong><\/p>

Marcelle<\/strong>Sans structures de gouvernance, il n'y a pas d'approche coordonn\u00e9e de la prescription. Les \u00e9quipes multidisciplinaires de gestion des antimicrobiens (GAMA) sont essentielles pour \u00e9laborer des interventions r\u00e9alistes et sp\u00e9cifiques au contexte. Nous avons recommand\u00e9 au Minist\u00e8re que les h\u00f4pitaux soient oblig\u00e9s de les former, et le Minist\u00e8re de la Sant\u00e9 a accept\u00e9. D'ici la fin de 2024, une directive a \u00e9t\u00e9 adress\u00e9e \u00e0 tous les h\u00f4pitaux, publics et priv\u00e9s. C'est un changement majeur.<\/p>

Comment vous \u00eates-vous d'abord impliqu\u00e9 dans drive-AMS ? Est-ce par l'interm\u00e9diaire de l'\u00e9quipe grecque de drive-AMS ?<\/strong><\/p>

Marcelle<\/strong>: Non. Cela a commenc\u00e9 avec Rosa Perran, du minist\u00e8re n\u00e9erlandais de la Sant\u00e9, du Bien-\u00eatre et des Sports. Lors d'une r\u00e9union EU-JAMRAI, elle a sugg\u00e9r\u00e9 que nous suivions le cours de formation drive-AMS. De l\u00e0, nous avons \u00e9t\u00e9 pr\u00e9sent\u00e9s \u00e0 Jeroen Schouten et plus tard au Prof. Roilides en Gr\u00e8ce.<\/p>

Linos :<\/strong> Nous avons r\u00e9alis\u00e9 que le cours correspondait parfaitement \u00e0 Chypre. Nous venions de mandater les comit\u00e9s AMS, et c'\u00e9tait une opportunit\u00e9 opportune pour les former.<\/p>

Qu'a permis la formation de f\u00e9vrier 2025 ?<\/strong><\/p>

Linos : <\/strong>La premi\u00e8re formation a rassembl\u00e9 les \u00e9quipes de l'AMS<\/a> des huit plus grands h\u00f4pitaux de Chypre : quatre publics, quatre priv\u00e9s. Chaque \u00e9quipe a con\u00e7u un projet r\u00e9aliste \u00e0 petite \u00e9chelle pour lancer son programme de gestion des antimicrobiens. C'\u00e9tait un excellent moyen de combiner la politique et la pratique.<\/p>

Marcella :<\/strong> Le cours nous a \u00e9galement initi\u00e9s \u00e0 de nouveaux concepts tels que l'identification des obstacles et des facilitateurs comportementaux. Cependant, un message important \u00e0 retenir est que le changement prend du temps. Il commence par des objectifs petits et r\u00e9alistes. Dans le cadre du cours, les formateurs ont utilis\u00e9 la m\u00e9taphore du bambou chinois : vous arrosez et arrosez et ne voyez rien pendant longtemps, mais soudain, il pousse fort et grand. C'est ainsi que le changement dans AMS prendra racine.<\/p>

Quel est le plan de suivi apr\u00e8s le cours de f\u00e9vrier ?<\/strong><\/p>

Linos<\/strong>Chaque h\u00f4pital a con\u00e7u un projet r\u00e9aliste de SMA durant le cours. Nous organisons maintenant des t\u00e9l\u00e9conf\u00e9rences de suivi tous les quelques mois o\u00f9 chaque \u00e9quipe pr\u00e9sente ses progr\u00e8s. Des formateurs de Gr\u00e8ce se joignent \u00e0 nous pour conseiller et soutenir.<\/p>

Marcella :<\/strong> La plupart des h\u00f4pitaux ont \u00e9galement effectu\u00e9 un Global-PPS<\/a> mesure avant le cours, et beaucoup pr\u00e9voient de le refaire dans un an pour suivre les progr\u00e8s. Nous avons trouv\u00e9 le Global-PPS tr\u00e8s convivial et pas trop long. C'est donc un excellent outil pour mesurer les progr\u00e8s au fil du temps.<\/p>

Quels r\u00e9sultats esp\u00e9rez-vous voir ?<\/strong><\/p>

Marcelle<\/strong>: Nous ne nous attendons pas \u00e0 ce que Chypre devienne la Su\u00e8de du jour au lendemain. Mais nous esp\u00e9rons stabiliser la situation et cr\u00e9er de petites victoires dans chaque h\u00f4pital. Il s'agit d'initier un changement culturel, d'accro\u00eetre la sensibilisation et de favoriser le changement de l'int\u00e9rieur. Les bases sont pos\u00e9es.
