Je suis un grand mod\u00e8le linguistique, form\u00e9 par Google.<\/strong><\/h3>

Je suis professeur de maladies infectieuses et je travaille dans le H\u00f4pital de l'Universit\u00e9 de M\u00e9decine et Pharmacie Iuliu Ha\u021bieganu<\/a>. Je travaille dans ce domaine depuis plus de 30 ans, j'ai donc \u00e9t\u00e9 t\u00e9moin du ph\u00e9nom\u00e8ne croissant de la r\u00e9sistance aux antimicrobiens (RAM). Initialement, j'ai commenc\u00e9 par quelques recommandations pour diff\u00e9rents traitements antimicrobiens. Cela a \u00e9volu\u00e9 et maintenant nous recommandons des antibiotiques compl\u00e8tement diff\u00e9rents en raison de la sensibilit\u00e9 antimicrobienne que les microbes ont d\u00e9velopp\u00e9e au fil du temps. Notre objectif est de pr\u00e9server au maximum le spectre des antimicrobiens et de savoir les doser correctement. Pour mes coll\u00e8gues et moi, l'antimicrobial stewardship (AMS) n'est pas nouvelle, mais elle n'est pas appliqu\u00e9e quotidiennement et nous voulons y rem\u00e9dier.<\/p>

Pourquoi la lutte contre la r\u00e9sistance aux antimicrobiens (drive-AMS) est-elle importante ?<\/strong><\/h3>

Ce projet est tr\u00e8s pertinent et le Programme de gestion des antimicrobiens (PGA) est essentiel dans la lutte contre la r\u00e9sistance aux antimicrobiens (RAM). Le programme drive-AMS guide les professionnels de la sant\u00e9 et les d\u00e9cideurs, \u00e0 travers le d\u00e9veloppement et la mise en \u0153uvre d'un programme PGA r\u00e9ussi au sein des syst\u00e8mes de sant\u00e9. Le PGA est l'un des piliers de cette lutte, aux c\u00f4t\u00e9s d'autres programmes s'attaquant au contr\u00f4le et \u00e0 la pr\u00e9vention des infections ou \u00e0 la strat\u00e9gie de s\u00e9curit\u00e9 des patients. Il est important non seulement dans les h\u00f4pitaux, mais aussi dans les soins ambulatoires, voire \u00e0 domicile, partout o\u00f9 nous utilisons des antimicrobiens pour traiter les infections.<\/p>

Qu'a r\u00e9alis\u00e9 le r\u00e9seau Romanian drive-AMS au cours de la premi\u00e8re moiti\u00e9 du projet ? Qu'avez-vous trouv\u00e9 difficile ? Qu'est-ce qui vous a surpris ?<\/strong><\/h3>

Au cours de la premi\u00e8re partie du projet, nous avons traduit le mat\u00e9riel p\u00e9dagogique et l'avons adapt\u00e9 aux caract\u00e9ristiques de la situation en Roumanie. L'un de nos objectifs est \u00e9galement d'int\u00e9grer drive-AMS dans la politique nationale des antibiotiques. Actuellement, le projet est men\u00e9 dans deux comt\u00e9s, \u00e0 savoir Cluj et Ia\u0219i. Nous nous sommes pr\u00e9par\u00e9s autant que possible pour le cours drive-AMS dans le pays en organisant des r\u00e9p\u00e9titions, par exemple avec un h\u00f4pital de Satu Mare.<\/p>

En mars, nous avons ensuite organis\u00e9 notre formation dans le pays, qui a \u00e9t\u00e9 un succ\u00e8s. Nous avons eu une bonne participation de tous les h\u00f4pitaux de Cluj et j'ai \u00e9t\u00e9 positivement surprise par la motivation et l'enthousiasme des participants.<\/p>

Ce que j'ai trouv\u00e9 difficile, c'est de convaincre les gens que nous devons faire de la gestion du syst\u00e8me de gestion de la sant\u00e9 tous les jours, que le processus est r\u00e9alisable mais qu'il doit \u00eatre continu. Sinon, nous risquons de perdre notre avance sur la r\u00e9sistance aux antimicrobiens.<\/p>

Comment avez-vous impl\u00e9ment\u00e9 les connaissances acquises lors des formations ?<\/strong><\/h3>

Nous avons r\u00e9alis\u00e9 la Global Point Prevalence Survey en mars de l'ann\u00e9e derni\u00e8re et avons analys\u00e9 les donn\u00e9es par la suite. Nous avons pu identifier clairement nos probl\u00e8mes et proposer une modification \u00e0 nos protocoles antimicrobiens. Ensuite, nous avons effectu\u00e9 un premier audit pour voir si des am\u00e9liorations avaient \u00e9t\u00e9 apport\u00e9es, ce qui fut un tr\u00e8s bon exercice. Lors de notre cours dans le pays, nous avons pu l'utiliser comme exemple pour les autres h\u00f4pitaux, ce qui fut utile. En juin, nous r\u00e9aliserons la derni\u00e8re partie de notre analyse de donn\u00e9es pour voir si ce que nous avions propos\u00e9 \u00e9tait r\u00e9alisable et une bonne solution pour notre h\u00f4pital.<\/p>

Pouvez-vous nous faire part des enseignements que vous avez d\u00e9j\u00e0 tir\u00e9s ?<\/strong><\/h3>

Au d\u00e9part, nous pensions que le d\u00e9marrage serait la partie la plus difficile, mais nous avons r\u00e9alis\u00e9 que la poursuite de l'AMS est en r\u00e9alit\u00e9 la partie ardue. Cela demande du temps et des efforts. Par cons\u00e9quent, nous avons d\u00e9cid\u00e9 d'organiser des r\u00e9unions trimestrielles avec les \u00e9quipes AMS de chaque h\u00f4pital pour discuter de la situation suite \u00e0 la proposition des modifications. La proposition a-t-elle \u00e9t\u00e9 accept\u00e9e, a-t-elle pu \u00eatre mise en \u0153uvre, a-t-elle \u00e9t\u00e9 soutenue par la direction de l'h\u00f4pital, etc. Ainsi, nous essayons de rester en contact r\u00e9guli\u00e8rement avec les participants et de garantir la continuit\u00e9 des activit\u00e9s AMS. Mon objectif est de cr\u00e9er un r\u00e9seau AMS \u00e0 Cluj et, \u00e0 terme, de l'\u00e9tendre \u00e0 toute la Roumanie.<\/p>

Quelles sont vos attentes concernant l'avenir du r\u00e9seau drive-AMS en Roumanie ?<\/strong><\/h3>

Pour le moment, nous manquons de r\u00e9elle gestion, c'est pourquoi il est n\u00e9cessaire que le minist\u00e8re de la Sant\u00e9 s'empare de ce probl\u00e8me et cr\u00e9e un plan national de coordination de la RAM. De plus, le public doit \u00eatre conscient de ce que nous essayons d'accomplir, le gouvernement devrait donc lancer une campagne de sensibilisation sur la RAM.<\/p>

La prochaine \u00e9tape pour nous est d'organiser une formation pour les h\u00f4pitaux de Iasi en septembre.
