Dans des \u00e9valuations pr\u00e9c\u00e9dentes, nous avons constat\u00e9 que, dans environ la moiti\u00e9 des situations o\u00f9 la ceftriaxone \u00e9tait utilis\u00e9e de mani\u00e8re empirique, il \u00e9tait possible de choisir de meilleures alternatives ayant un impact moindre sur la r\u00e9sistance aux antimicrobiens \u201d, explique le Dr. Almeida.<\/p><p>\u201c Cette situation \u00e9tait particuli\u00e8rement fr\u00e9quente dans le traitement de la pneumonie communautaire, o\u00f9 la ceftriaxone, qui ne fait pas partie des traitements de premi\u00e8re intention selon le protocole local, est utilis\u00e9e de mani\u00e8re empirique dans 20% des cas. Nous avons donc cherch\u00e9 \u00e0 \u00e9tablir une collaboration avec le service de m\u00e9decine interne (o\u00f9 la majorit\u00e9 des cas de pneumonie communautaire sont hospitalis\u00e9s) afin de r\u00e9duire ce chiffre d\u2019au moins 50 % \u201d, ajoute le Dr Almeida.<\/p><p>La strat\u00e9gie d\u00e9finie impliquait une \u00e9valuation initiale des obstacles et des facilitateurs de l'intervention ainsi que l'obtention de donn\u00e9es de qualit\u00e9 sur l'utilisation des antibiotiques dans ce contexte. Ensuite, des visites ont \u00e9t\u00e9 effectu\u00e9es lors des r\u00e9unions des services de m\u00e9decine interne pour partager les donn\u00e9es de prescription obtenues, discuter de ce probl\u00e8me, renforcer les comportements positifs et cr\u00e9er une culture de bonne prescription. L'intervention a \u00e9t\u00e9 con\u00e7ue avec le Dr Nuno Pereira et le Professeur Jos\u00e9 Artur Paiva, avec une collaboration importante du Dr Lucybell Moreira et du Professeur Ana Azevedo dans le d\u00e9veloppement de la strat\u00e9gie de collecte de donn\u00e9es.<\/p><p>\u201c \u00c0 la suite de cette intervention, le service de m\u00e9decine interne a r\u00e9ussi \u00e0 r\u00e9duire la proportion de personnes atteintes de pneumonie dans la communaut\u00e9 et trait\u00e9es \u00e0 la ceftriaxone, qui est pass\u00e9e d\u2019une valeur initiale de 20% \u00e0 une m\u00e9diane de 6,7% \u201d, pr\u00e9cise-t-il.<\/p><p>Le certificat en gestion des antimicrobiens organis\u00e9 par <a href=\"https:\/\/www.escmid.org\/\" target=\"_blank\" rel=\"noopener\">ESCMID<\/a>, est un projet de formation qui a dur\u00e9 deux ans (2022-2024) et auquel ont particip\u00e9 environ 40 m\u00e9decins exp\u00e9riment\u00e9s dans l'am\u00e9lioration de la prescription des antimicrobiens. Il comprenait 10 cours courts qui se sont d\u00e9roul\u00e9s dans plusieurs pays europ\u00e9ens au cours de ces deux ann\u00e9es, en plus du d\u00e9veloppement d'une intervention d'am\u00e9lioration de la qualit\u00e9 au niveau local, guid\u00e9e par les donn\u00e9es et ax\u00e9e sur la modification des aspects comportementaux qui r\u00e9gissent la prescription d'antibiotiques.<\/p><p>\u201c Cette distinction repr\u00e9sente une grande satisfaction personnelle car elle marque l'aboutissement de deux ann\u00e9es d'apprentissage et d'interactions enrichissantes avec d'autres professionnels du domaine.
