De plus, je fais partie de l'\u00e9quipe drive-AMS, je r\u00e9alise donc l'enqu\u00eate sur la pr\u00e9valence ponctuelle pour mon h\u00f4pital. Le professeur Mihaela Lupse et moi mettons en \u0153uvre ce que nous avons appris du cours drive-AMS et pr\u00e9parons notre cours drive-AMS national pour les h\u00f4pitaux de Roumanie, qui aura lieu en mars.<\/p><h3><strong>Que fait un pharmacien clinicien au quotidien ? <\/strong><\/h3><p>Je fais des rondes quotidiennes pour v\u00e9rifier les m\u00e9dicaments des patients, mais je ne peux pas voir tous les patients car je suis le seul pharmacien clinicien dans mon h\u00f4pital. C'est pourquoi j'alterne entre les services pour m'assurer que je visite r\u00e9guli\u00e8rement chacun d'eux. J'examine les plans de traitement et les ordonnances et je m'assure qu'il n'y a pas d'interactions et de contre-indications pour les m\u00e9dicaments prescrits, ni qu'il y a lieu d'arr\u00eater les antibiotiques.<\/p><h3><strong>Comment menez-vous une gestion appropri\u00e9e des antimicrobiens dans vos h\u00f4pitaux ? <\/strong><\/h3><p>Nous avons des protocoles que nous essayons de suivre pour les maladies infectieuses les plus fr\u00e9quentes dans notre h\u00f4pital. Nous avons \u00e9galement une liste d'antibiotiques restreints qui ne peuvent \u00eatre prescrits qu'apr\u00e8s approbation. Nous suivons la consommation d'antibiotiques et les sch\u00e9mas de r\u00e9sistance, examinons les rapports que nous recevons du laboratoire et modifions les protocoles en cons\u00e9quence.<\/p><h3><strong>Pouvez-vous me donner quelques exemples concrets de vos activit\u00e9s AMS ?<\/strong><\/h3><p>Les rapports Global-PPS ont montr\u00e9 que le probl\u00e8me le plus important dans notre h\u00f4pital est la surconsommation de carbap\u00e9n\u00e8mes, c'est ce que nous visons \u00e0 r\u00e9duire actuellement. De plus, nous travaillons \u00e0 la mise en place d'un protocole pour les infections invasives \u00e0 candida.<\/p><h3><strong>Dans votre travail quotidien, quels sont les principaux d\u00e9fis ou biais auxquels vous \u00eates confront\u00e9 en mati\u00e8re de RAM ?<\/strong><\/h3><p>En Roumanie, nous sommes confront\u00e9s \u00e0 une utilisation excessive des antibiotiques. De nombreux patients prennent des antibiotiques m\u00eame sans ordonnance, ce qui entra\u00eene des taux de r\u00e9sistance tr\u00e8s \u00e9lev\u00e9s. Il existe notamment de nombreuses bact\u00e9ries multir\u00e9sistantes et les antibiotiques pour ce type ne sont pas largement disponibles. Nous avons constat\u00e9 qu'il y avait beaucoup plus de cas de patients atteints d'infections multir\u00e9sistantes en 2023 par rapport aux ann\u00e9es pr\u00e9c\u00e9dentes. La plupart de ces patients viennent dans notre h\u00f4pital car nous sommes sp\u00e9cialis\u00e9s dans les maladies infectieuses, ce qui repr\u00e9sente une charge importante.<\/p><p>Actuellement, il y a un manque de sensibilisation autour de l'antimicrobien r\u00e9sistance et de la r\u00e9sistance aux antimicrobiens en Roumanie.
