Les cas de maladies fongiques invasives (MFI) sont en augmentation, en particulier chez les populations \u00e0 risque et immunod\u00e9prim\u00e9es. Le Centre europ\u00e9en de pr\u00e9vention et de contr\u00f4le des maladies (ECDC) a r\u00e9cemment signal\u00e9 une augmentation du nombre de cas et d'\u00e9pid\u00e9mies caus\u00e9s par un pathog\u00e8ne critique, <em>Candida auris, <\/em>dans l'UE\/EEE entre 2020 et 2021 (Anke Kohlenberg, Euro Surveill. 2022).<\/p><p>Pour renforcer la r\u00e9ponse mondiale aux IFD et \u00e0 la r\u00e9sistance aux antifongiques, le <a href=\"https:\/\/www.who.int\/\" target=\"_blank\" rel=\"noopener\">Organisation Mondiale de la Sant\u00e9<\/a> (OMS) a d\u00e9velopp\u00e9 un <a href=\"https:\/\/www.who.int\/publications\/i\/item\/9789240060241\" target=\"_blank\" rel=\"noopener\">Liste des agents pathog\u00e8nes fongiques prioritaires<\/a> (FPPL). Son objectif est de concentrer et de stimuler davantage la recherche et les interventions politiques. Cette liste s'est inspir\u00e9e de la liste des pathog\u00e8nes bact\u00e9riens prioritaires publi\u00e9e par l'OMS en 2017.<\/p><p>La FPPL est le premier effort mondial visant \u00e0 hi\u00e9rarchiser syst\u00e9matiquement les agents pathog\u00e8nes fongiques, soulignant ainsi leurs besoins non satisfaits en mati\u00e8re de recherche et d\u00e9veloppement et le manque de perception de leur importance pour la sant\u00e9 publique. L'OMS propose trois domaines d'action et de mise en \u0153uvre de la FPPL :<\/p><ol><li><strong>actions de surveillance<\/strong>, interventions et strat\u00e9gies, y compris la quantification du fardeau des infections fongiques invasives et de la r\u00e9sistance aux antifongiques, et le renforcement des capacit\u00e9s diagnostiques en mycologie<\/li><li><strong>investissements durables en R&amp;D et en innovation<\/strong> y compris l'am\u00e9lioration des th\u00e9rapies existantes et le soutien \u00e0 la recherche pour des diagnostics rapides et pr\u00e9cis<\/li><li><strong>interventions de sant\u00e9 publique <\/strong>y compris la promotion de mesures de pr\u00e9vention et de contr\u00f4le des infections et la garantie d'un acc\u00e8s \u00e9quitable et abordable \u00e0 des agents antifongiques de qualit\u00e9.<\/li><\/ol><p>Les agents pathog\u00e8nes sont divis\u00e9s en trois cat\u00e9gories : priorit\u00e9 critique, \u00e9lev\u00e9e et moyenne.<\/p><p><a href=\"https:\/\/www.global-pps.com\/wp-content\/uploads\/2023\/01\/Screenshot-2023-01-11-161648.png\"><img decoding=\"async\" class=\"alignnone wp-image-15788\" src=\"https:\/\/www.global-pps.com\/wp-content\/uploads\/2023\/01\/Screenshot-2023-01-11-161648.png\" alt=\"\" width=\"400\" height=\"313\" srcset=\"https:\/\/www.global-pps.com\/wp-content\/uploads\/2023\/01\/Screenshot-2023-01-11-161648.png 712w, https:\/\/www.global-pps.com\/wp-content\/uploads\/2023\/01\/Screenshot-2023-01-11-161648-300x235.png 300w, https:\/\/www.global-pps.com\/wp-content\/uploads\/2023\/01\/Screenshot-2023-01-11-161648-16x12.png 16w\" sizes=\"(max-width: 400px) 100vw, 400px\" \/><\/a><\/p><p>Depuis 2015, <strong>l'outil de surveillance standardis\u00e9 Global-PPS<\/strong> collecte des donn\u00e9es sur les sch\u00e9mas de prescription des antimycotiques (ATC J02) et des antifongiques (ATC D01B) \u00e0 usage syst\u00e9mique dans les h\u00f4pitaux du monde entier. De plus, depuis 2019, des donn\u00e9es de r\u00e9sistance sur <em>Candida spp.<\/em>., <em>Aspergillus spp<\/em>. et d'autres champignons ont \u00e9t\u00e9 collect\u00e9s. En r\u00e9ponse \u00e0 la publication de la liste des FPM de l'OMS, nous avons examin\u00e9 les pratiques de prescription des antifongiques et antimycotiques dans le monde. Les r\u00e9sultats montrent des taux globaux \u00e9lev\u00e9s d'utilisation empirique et prophylactique, y compris pour les agents de deuxi\u00e8me ligne, soulignant la n\u00e9cessit\u00e9 de renforcer les interventions de gestion des traitements antifongiques, surtout face \u00e0 la r\u00e9sistance croissante.
