1709w\" sizes=\"(max-width: 300px) 100vw, 300px\" \/>Le programme CwPAMS, financ\u00e9 par le Fleming Fund de l'aide britannique et g\u00e9r\u00e9 conjointement par le Tropical Health and Education Trust (THET) et la Commonwealth Pharmacists Association (CPA), a soutenu 12 partenariats de sant\u00e9 dans quatre pays africains (Tanzanie, Zambie, Ouganda et Ghana). L'objectif de l'\u00e9tude \u00e9tait de cr\u00e9er une liste de contr\u00f4le contextualis\u00e9e des \u00e9l\u00e9ments cl\u00e9s de l'AMU et des comportements essentiels \u00e0 utiliser dans les \u00e9tablissements de sant\u00e9 en Afrique subsaharienne, et de mettre en \u0153uvre cette liste de contr\u00f4le dans les institutions de sant\u00e9 des quatre pays africains.<\/p><h2>D\u00e9veloppement et d\u00e9ploiement de la liste de contr\u00f4le CwPAMS<\/h2><p>Utilisant <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/29625170\/\" target=\"_blank\" rel=\"noopener\">Pulcini et al.<\/a> En partant d'une liste de contr\u00f4le AMS mondiale, une proc\u00e9dure Delphi modifi\u00e9e a \u00e9t\u00e9 adopt\u00e9e pour le contexte des soins de sant\u00e9 en Afrique subsaharienne. Quatorze repr\u00e9sentants de la sant\u00e9 des partenariats CwPAMS de Tanzanie, de Zambie, d'Ouganda, du Ghana et du Royaume-Uni ont \u00e9t\u00e9 invit\u00e9s \u00e0 examiner, noter et commenter les \u00e9l\u00e9ments de la liste de contr\u00f4le. La liste de contr\u00f4le AMS finalis\u00e9e a \u00e9t\u00e9 d\u00e9ploy\u00e9e sous forme de formulaire en ligne dans 19 sites de soins de sant\u00e9 ; elle contenait 54 \u00e9l\u00e9ments couvrant principalement les \u00e9l\u00e9ments essentiels des programmes AMS hospitaliers.<\/p><p>Des interventions de renforcement de l'AMS ont eu lieu dans 14 partenariats CwPAMS, apr\u00e8s quoi la liste de contr\u00f4le a \u00e9t\u00e9 compl\u00e9t\u00e9e pour la deuxi\u00e8me fois ; celles-ci comprenaient \u00e9galement des questions ouvertes sur les activit\u00e9s suppl\u00e9mentaires auxquelles ils avaient particip\u00e9 sur la base de la liste de contr\u00f4le (r\u00e9sultats post\u00e9rieurs).<\/p><h2>Am\u00e9liorations quantitatives suite aux interventions de l'AMS<\/h2><p>Sur ces 14 sites CwPAMS, des am\u00e9liorations post-intervention ont \u00e9t\u00e9 observ\u00e9es pour tous les composants de l'AMS, fournies gr\u00e2ce aux partenariats. Ces am\u00e9liorations de l'AMS comprenaient la participation \u00e0 la formation suivie de la mise en \u0153uvre de l'enqu\u00eate mondiale sur la pr\u00e9valence ponctuelle (<a href=\"https:\/\/www.global-pps.com\/fr\/\" target=\"_blank\" rel=\"noopener\">Global-PPS<\/a>) (11 h\u00f4pitaux sur 14 ; am\u00e9lioration de 721 points TP3T). Un autre aspect qui s&#x27;est consid\u00e9rablement am\u00e9lior\u00e9 est la mise en place de structures multidisciplinaires formelles d\u00e9di\u00e9es \u00e0 la prise en charge des maux de t\u00eate aigus, avec une am\u00e9lioration de 791 points TP3T apr\u00e8s l&#x27;intervention. Les \u00e9l\u00e9ments ayant le moins progress\u00e9 sont l&#x27;acc\u00e8s aux services de laboratoire et d&#x27;imagerie (71 points TP3T) et la mise en place d&#x27;un soutien financier ad\u00e9quat pour la prise en charge des maux de t\u00eate aigus (141 points TP3T).<\/p><p>Les auteurs concluent que les outils de gestion de l'antimicrobiens (AMS) contextualis\u00e9s et co-cr\u00e9\u00e9s sont essentiels pour g\u00e9rer l'usage des antimicrobiens dans les \u00e9tablissements de sant\u00e9 et pour renforcer l'appropriation et l'engagement locaux en mati\u00e8re d'AMS. Cette \u00e9tude a conduit au d\u00e9veloppement d'une nouvelle liste de contr\u00f4le AMS, qui s'est av\u00e9r\u00e9e efficace pour capturer les am\u00e9liorations de l'AMS dans les quatre pays africains.
