canadiens a publi\u00e9 un article dans CMAJ Open, rendant compte de leurs r\u00e9sultats du Global-PPS. Cette \u00e9tude d\u00e9crit l'utilisation des antimicrobiens (UAM) bas\u00e9e sur des donn\u00e9es d\u00e9taill\u00e9es au niveau du patient dans les h\u00f4pitaux canadiens en 2018 en termes de pr\u00e9valence des antimicrobiens, d'indications et de s\u00e9lection des agents dans les unit\u00e9s m\u00e9dicales, chirurgicales et de soins intensifs pour adultes et enfants. De plus, elle \u00e9value la qualit\u00e9 de la prescription des antimicrobiens \u00e0 l'aide d'un ensemble d'indicateurs cl\u00e9s de qualit\u00e9.<\/p><p><strong>Une strat\u00e9gie pancanadienne<\/strong><\/p><p>En 2017, le Canada a publi\u00e9 la Strat\u00e9gie pour la lutte contre la r\u00e9sistance aux antimicrobiens et l'usage des antimicrobiens : <a href=\"https:\/\/www.canada.ca\/en\/health-canada\/services\/publications\/drugs-health-products\/tackling-antimicrobial-resistance-use-pan-canadian-framework-action.html\" target=\"_blank\" rel=\"noopener\">Un cadre d'action pancanadien<\/a> pour renforcer sa strat\u00e9gie sur la r\u00e9sistance aux antimicrobiens. La surveillance de la consommation d'antimicrobiens est un \u00e9l\u00e9ment central du cadre d'action du Canada et c'est pourquoi le Global-PPS a \u00e9t\u00e9 utilis\u00e9 pour obtenir ces donn\u00e9es.<\/p><p><strong>Quels ont \u00e9t\u00e9 les r\u00e9sultats ?<\/strong><\/p><p>Cette \u00e9tude a mobilis\u00e9 47 h\u00f4pitaux, pour un total de 13 272 patients. Au total, 6 525 ordonnances ont \u00e9t\u00e9 examin\u00e9es. La pr\u00e9valence globale des AMU s\u2019\u00e9levait \u00e0 33,51 % : 74,11 % \u00e9taient utilis\u00e9es \u00e0 des fins th\u00e9rapeutiques, 12,61 % pour la prophylaxie m\u00e9dicale et 8,91 % pour la prophylaxie chirurgicale. Parmi toutes les prescriptions th\u00e9rapeutiques, 61% ont \u00e9t\u00e9 utilis\u00e9es de mani\u00e8re empirique et 39% \u00e9taient cibl\u00e9es. Les cat\u00e9gories diagnostiques d&#x27;utilisation les plus fr\u00e9quentes concernaient les infections des voies respiratoires (29,61 %), les infections des voies urinaires (11,61 %) et les infections intra-abdominales (11,1 %).  Trente-huit pour cent ont \u00e9t\u00e9 utilis\u00e9s pour des infections nosocomiales.  Le traitement cibl\u00e9 pour les MDRO document\u00e9es \u00e9tait relativement peu fr\u00e9quent, puisque seuls 82 patients pr\u00e9sentaient une infection \u00e0 SARM document\u00e9e, 101 une r\u00e9sistance \u00e0 3<sup>rd<\/sup> c\u00e9phalosporines de troisi\u00e8me g\u00e9n\u00e9ration Ent\u00e9robact\u00e9ries et 30 avec r\u00e9sistance aux carbap\u00e9n\u00e8mes (5 produisant des carbap\u00e9n\u00e9mases). <em>\u00a0<\/em>Le diagnostic a \u00e9t\u00e9 consign\u00e9 dans le dossier du patient au moment de la mise en place du traitement antimicrobien (87,31 %). Par ailleurs, l&#x27;\u00e9tude a montr\u00e9 que 62,91 % des traitements antimicrobiens comportaient une date d&#x27;arr\u00eat ou de r\u00e9\u00e9valuation et que 72,01 % des prescriptions respectaient les recommandations locales.<\/p><p><strong>Conclusion<\/strong><\/p><p>Voici le plus vaste projet de surveillance de l'utilisation et de la r\u00e9sistance aux antimicrobiens au Canada. L'\u00e9tude fournit des renseignements valides et fiables sur les pratiques de prescription d'antimicrobiens et cerne des domaines \u00e0 am\u00e9liorer dans les h\u00f4pitaux canadiens participants. L'utilisation globale d'antimicrobiens est similaire \u00e0 ce qui a \u00e9t\u00e9 d\u00e9clar\u00e9 pr\u00e9c\u00e9demment au Canada, et confirme que le traitement des MDRO est faible.<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/32653046\/\" target=\"_blank\" rel=\"noopener\">C. Frenette et al., Antimicrob Resist Infect Control, 2020<\/a>. Deux constatations sugg\u00e8rent que la prescription d'antimicrobiens devient plus rationnelle\u00a0:<\/p><ul><li>La pr\u00e9valence de l'usage des antimicrobiens chez les patients p\u00e9diatriques et n\u00e9onatals semble diminuer<\/li><li>La prophylaxie chirurgicale en dose unique a \u00e9t\u00e9 privil\u00e9gi\u00e9e par rapport \u00e0 la prophylaxie chirurgicale de plus d'une journ\u00e9e. Le respect des lignes directrices sur la prophylaxie chirurgicale \u00e9tait \u00e9lev\u00e9 parmi les h\u00f4pitaux interrog\u00e9s, bien qu'il soit plus faible dans les provinces de l'Atlantique. De nouveaux protocoles ont depuis \u00e9t\u00e9 \u00e9labor\u00e9s dans cette r\u00e9gion.<\/li><\/ul><p>Les r\u00e9sultats servent d'\u00e9talon de r\u00e9f\u00e9rence g\u00e9n\u00e9ral pour les programmes de g\u00e9rance nationaux et locaux.
