l'H\u00f4pital \u00c9vang\u00e9lique de Vanga depuis juillet 2021.<\/strong><\/em><\/p><p>La R\u00e9publique D\u00e9mocratique du Congo (RDC) est un pays post-conflit confront\u00e9 \u00e0 de nombreux d\u00e9fis sanitaires : un syst\u00e8me de sant\u00e9 faible, des ressources humaines limit\u00e9es, des facteurs g\u00e9ographiques (taille du pays, zones foresti\u00e8res) et, par cons\u00e9quent, un acc\u00e8s difficile aux soins de sant\u00e9.<\/p><p><strong>Contexte \u2013 Comment \u00e7a a commenc\u00e9<\/strong><\/p><p>Depuis longtemps, on observe une surconsommation et une utilisation inappropri\u00e9e \u00e0 la fois des antibiotiques et des antipaludiques. Cela constitue une menace pour la situation financi\u00e8re de cet h\u00f4pital rural. Le probl\u00e8me est connu, mais le changement de comportement est difficile.<\/p><p>Pour aborder cette situation, j'ai d'abord d\u00fb documenter la situation actuelle par la collecte de donn\u00e9es sur l'utilisation des antimicrobiens. La collecte de donn\u00e9es de r\u00e9f\u00e9rence permettrait de mesurer l'efficacit\u00e9 de toute intervention future. Je suis all\u00e9 sur Internet et j'ai recherch\u00e9 des moyens de r\u00e9soudre ce probl\u00e8me. Heureusement, j'ai d\u00e9couvert le <a href=\"http:\/\/www.global-pps.com\/fr\/\" target=\"_blank\" rel=\"noopener\">Global-PPS<\/a>, un outil qui offrait exactement ce que je cherchais. Avec le soutien du directeur de l'h\u00f4pital et du personnel m\u00e9dical, nous avons commenc\u00e9 la mise en \u0153uvre.<\/p><p><strong>Collecte de donn\u00e9es : Le premier d\u00e9fi<\/strong><\/p><p>Avec mon \u00e9quipe, nous avons discut\u00e9 du processus du Global-PPS. Nous nous sommes entra\u00een\u00e9s trois fois avant de commencer la collecte de donn\u00e9es r\u00e9elle. Chacun des exercices nous a aid\u00e9s \u00e0 identifier les probl\u00e8mes potentiels. Nous en avons discut\u00e9 avec l'\u00e9quipe Global-PPS en Belgique. Ils ont r\u00e9pondu tr\u00e8s rapidement et nous ont aid\u00e9s \u00e0 r\u00e9soudre toutes les difficult\u00e9s rencontr\u00e9es. Apr\u00e8s la collecte des donn\u00e9es, j'ai saisi les donn\u00e9es sur l'application en ligne de saisie des donn\u00e9es. J'ai imm\u00e9diatement re\u00e7u le rapport final. Ce fut une bonne exp\u00e9rience pour moi et mon \u00e9quipe.<\/p><p><strong>Pr\u00e9sentation des r\u00e9sultats<\/strong><\/p><p>Pour pr\u00e9senter les r\u00e9sultats, nous avons invit\u00e9 toutes les personnes impliqu\u00e9es dans le processus de prescription \u00e0 l'h\u00f4pital ainsi que les chefs de service. Nous avons mis en \u00e9vidence la mauvaise utilisation des antimicrobiens, principalement des antibiotiques, dans les services p\u00e9diatrique, chirurgical et obst\u00e9trical de l'h\u00f4pital. Nous avons re\u00e7u des f\u00e9licitations de la part des participants \u00e0 cette r\u00e9union avant d'aborder la discussion sur l'intervention.<\/p><p><strong>Cr\u00e9ation d'un comit\u00e9 de gestion des antimicrobiens<\/strong><\/p><p>Nous avons d\u00e9cid\u00e9 de constituer une \u00e9quipe qui travaille avec moi au sein du comit\u00e9 de gestion des antimicrobiens. Ce comit\u00e9 est compos\u00e9 de m\u00e9decins, d'infirmi\u00e8res et d'administrateurs. Nous avons constat\u00e9 que l'absence d'un laboratoire (qui pourrait aider avec les cultures pour identifier le germe et ajuster le traitement), l'absence de directives locales de prescription d'antibiotiques et l'absence de protocoles de nettoyage \u00e0 l'h\u00f4pital \u00e9taient les principaux facteurs de mauvaise utilisation des antibiotiques.<\/p><p><strong>Nous avons \u00e9labor\u00e9 les points d'action suivants :<\/strong><\/p><ul><li>\u00c9laborer une politique d'antibiotiques locale et en superviser la mise en \u0153uvre. Nous avons commenc\u00e9 par l'\u00e9laboration de lignes directrices sur la prophylaxie chirurgicale afin de standardiser ces prescriptions. L'absence de tests de sensibilit\u00e9 reste un probl\u00e8me pour nous guider dans l'\u00e9laboration de lignes directrices pour les traitements antibiotiques empiriques.<\/li><li>\u00c9laborer un programme de nettoyage \u00e0 l'h\u00f4pital. Lorsque la plupart des membres du personnel craignent la capacit\u00e9 infectieuse d'un environnement sale, ils ont tendance \u00e0 prescrire syst\u00e9matiquement des antibiotiques. Nous mettons en place une unit\u00e9 de nettoyage bien organis\u00e9e \u00e0 l'h\u00f4pital avec du mat\u00e9riel adapt\u00e9.<strong>. <\/strong><\/li><li>Reformer le personnel et les patients au lavage des mains.
