Mi inter\u00e9s en la salud global comenz\u00f3 temprano: durante la facultad de medicina, hice una rotaci\u00f3n cl\u00ednica en Malawi. Luego, complet\u00e9 un doctorado sobre la malaria en el embarazo en el Royal Tropical Institute y el Amsterdam Medical Centre, que incluy\u00f3 visitas de investigaci\u00f3n a Burkina Faso y Gambia. As\u00ed fue como me involucr\u00e9 en la iniciativa SPICE.<\/p><p><em>\u00bfQu\u00e9 te motiv\u00f3 a emprender la misi\u00f3n SPICE en \u00c1frica?<\/em><\/p><p>Siempre me ha interesado el trabajo colaborativo, especialmente cuando se basa en las fortalezas de los profesionales locales. Lo que realmente me gusta de drive-AMS es su enfoque de mentor\u00eda, es decir, apoyar a los equipos locales en el desarrollo de sus propias intervenciones y la mejora de los proyectos de AMS. No se trata de imponer soluciones, sino de potenciar lo que ya existe. Ese tipo de asociaci\u00f3n es muy gratificante.<\/p><p><em>Est\u00e1s preparando una misi\u00f3n SPICE en Tanzania. \u00bfPuedes contarnos m\u00e1s sobre lo que eso implica, qui\u00e9n est\u00e1 involucrado y los desaf\u00edos que est\u00e1s abordando?<\/em><\/p><p>S\u00ed, la visita de campo a Tanzania est\u00e1 programada para este oto\u00f1o. Hasta ahora, he estado trabajando estrechamente con el equipo local a trav\u00e9s de reuniones peri\u00f3dicas para aclarar los problemas principales que enfrentan y para intercambiar ideas sobre posibles intervenciones. El enfoque ha estado en mejorar las pr\u00e1cticas de hemocultivo en su UCIN neonatal, donde han reportado altas tasas de contaminaci\u00f3n. Otro desaf\u00edo es determinar c\u00f3mo manejar casos en los que no est\u00e1 claro si el resultado de un hemocultivo es un contaminante, especialmente en neonatos, donde las decisiones cl\u00ednicas pueden ser complejas. Estamos trabajando en el desarrollo de un protocolo de manejo para guiar las decisiones de tratamiento en tales casos. Para este proyecto, soy el \u00fanico experto de Radboud, aunque consulto a colegas cuando es necesario. Si bien el n\u00facleo de esta misi\u00f3n es la gesti\u00f3n de diagn\u00f3sticos, a veces surgen preguntas relacionadas con la PCI. En esos casos, me pongo en contacto con colegas con experiencia en PCI para asegurarme de que estamos brindando la mejor orientaci\u00f3n posible. Tambi\u00e9n hemos presentado un plan de proyecto detallado a SPICE, que describe nuestros objetivos, cronograma y planes para la visita de campo. Estar en el sitio ser\u00e1 crucial, no solo para validar nuestros planes sino para obtener una comprensi\u00f3n m\u00e1s profunda del contexto local. Las reuniones remotas son \u00fatiles, pero no pueden reemplazar las ideas que se obtienen al ver c\u00f3mo funcionan las cosas sobre el terreno. Por ejemplo, en Tanzania, los residentes con frecuencia toman muestras de sangre, y su rotaci\u00f3n afecta la consistencia. La capacitaci\u00f3n se vuelve crucial. Tambi\u00e9n estoy ansioso por aprender m\u00e1s sobre los recursos disponibles, por eso la visita al sitio es tan importante.
