Los casos de enfermedades f\u00fangicas invasivas (EFI) van en aumento, especialmente entre poblaciones en riesgo e inmunocomprometidas. El Centro Europeo para la Prevenci\u00f3n y el Control de las Enfermedades (ECDC) inform\u00f3 recientemente un n\u00famero creciente de casos y brotes causados por un pat\u00f3geno cr\u00edtico, <em>Candida auris, <\/em>en la UE\/EEE durante 2020-2021 (Anke Kohlenberg, Euro Surveill. 2022).<\/p><p>Para fortalecer la respuesta mundial a las FAS y la resistencia a los antif\u00fangicos, la <a href=\"https:\/\/www.who.int\/\" target=\"_blank\" rel=\"noopener\">Organizaci\u00f3n Mundial de la Salud<\/a> (OMS) ha desarrollado un <a href=\"https:\/\/www.who.int\/publications\/i\/item\/9789240060241\" target=\"_blank\" rel=\"noopener\">Lista de Pat\u00f3genos F\u00fangicos Prioritarios<\/a> (FPPL). Su objetivo es centrar e impulsar una mayor investigaci\u00f3n e intervenciones pol\u00edticas. Esta lista se inspir\u00f3 en la Lista de Pat\u00f3genos Prioritarios Bacterianos que la OMS public\u00f3 en 2017.<\/p><p>El FPPL es el primer esfuerzo global para priorizar sistem\u00e1ticamente los pat\u00f3genos f\u00fangicos y resaltar as\u00ed sus necesidades de investigaci\u00f3n y desarrollo insatisfechas y la falta de importancia percibida para la salud p\u00fablica. La OMS propone tres \u00e1reas de acci\u00f3n e implementaci\u00f3n del FPPL:<\/p><ol><li><strong>acciones de vigilancia<\/strong>, intervenciones y estrategias que incluyen la cuantificaci\u00f3n de la carga de las IFD y la resistencia a los antif\u00fangicos y el desarrollo de la capacidad de diagn\u00f3stico micol\u00f3gico<\/li><li><strong>inversiones sostenibles en I+D e innovaci\u00f3n<\/strong> incluyendo la mejora de terapias existentes y el apoyo a la investigaci\u00f3n para diagn\u00f3sticos r\u00e1pidos y precisos<\/li><li><strong>intervenciones de salud p\u00fablica <\/strong>incluyendo la promoci\u00f3n de medidas de prevenci\u00f3n y control de infecciones y la garant\u00eda de un acceso equitativo y asequible a agentes antif\u00fangicos de calidad.<\/li><\/ol><p>Los pat\u00f3genos se dividen en tres categor\u00edas: prioridad cr\u00edtica, alta y media.<\/p><p><a href=\"https:\/\/www.global-pps.com\/wp-content\/uploads\/2023\/01\/Screenshot-2023-01-11-161648.png\"><img decoding=\"async\" class=\"alignnone wp-image-15788\" src=\"https:\/\/www.global-pps.com\/wp-content\/uploads\/2023\/01\/Screenshot-2023-01-11-161648.png\" alt=\"\" width=\"400\" height=\"313\" srcset=\"https:\/\/www.global-pps.com\/wp-content\/uploads\/2023\/01\/Screenshot-2023-01-11-161648.png 712w, https:\/\/www.global-pps.com\/wp-content\/uploads\/2023\/01\/Screenshot-2023-01-11-161648-300x235.png 300w, https:\/\/www.global-pps.com\/wp-content\/uploads\/2023\/01\/Screenshot-2023-01-11-161648-16x12.png 16w\" sizes=\"(max-width: 400px) 100vw, 400px\" \/><\/a><\/p><p>Desde 2015, <strong>la herramienta estandarizada de vigilancia Global-PPS<\/strong> recopila datos sobre los patrones de prescripci\u00f3n de antimic\u00f3ticos (ATC J02) y antif\u00fangicos (ATC D01B) para uso sist\u00e9mico en hospitales de todo el mundo. Adicionalmente, desde 2019, datos de resistencia sobre <em>Candida spp<\/em>., <em>Aspergillus spp<\/em>. y otros hongos se han recolectado. En respuesta a la publicaci\u00f3n de la OMS sobre FPLP, examinamos las pr\u00e1cticas de prescripci\u00f3n de antif\u00fangicos y antimic\u00f3ticos en todo el mundo. Los resultados muestran altas tasas generales de uso emp\u00edrico y profil\u00e1ctico, incluyendo agentes de segunda l\u00ednea, lo que destaca la necesidad de fortalecer las intervenciones de administraci\u00f3n para los tratamientos antif\u00fangicos, especialmente ante la creciente resistencia.
